Imaging files in DrOnlineIQ: what the viewer opens, what it refuses, who may release
The imaging module stores DICOM, PNG and JPEG files privately and opens uncompressed DICOM, JPEG-baseline DICOM, PNG and JPEG in its own viewer. Any other DICOM file is kept and offered as a download of the original. Only doctors write, sign, amend and release reports. It is not a PACS or a diagnostic workstation, and it reads nothing automatically.

Who it serves and what it solves
Owners deciding what the clinic can safely accept
A file is accepted only when its content is DICOM, PNG or JPEG, whatever its name says. SVG, HTML, PDF and other types are refused, and a damaged DICOM file is refused.
Clinics that receive exported images from outside
Files that arrive on a disc or as a download can be uploaded to a study and opened. The common uncompressed and JPEG-baseline formats display; for the rest, the original stays available.
Anyone responsible for patient privacy
The name, ID and birth date written inside a DICOM file are kept for staff only and are never shown or read in the patient portal. A person, not the file, decides which patient a study belongs to.
Clinics that want to control what a patient receives
A patient sees only a signed, current version that a doctor released. Images are a separate choice, off until the owner switches them on, and the doctor chooses them release by release.
Who does what
- Reception
- Accounts with the reception role have no access to imaging. Assistants can view, download, register studies and upload, but cannot remove files.
- Practitioner
- Is the only role that writes, signs, amends and releases a report, and the only one that confirms that a file's header identity is the chart patient.
- Cashier
- Has no access to imaging.
- Owner
- Holds the switch for releasing images to patients, can remove files and cancel studies, and cannot sign unless the account has the doctor role.
What the product does today
Upload checks: the type is read from the bytes, a DICOM file must parse, a picture must be a readable PNG or JPEG of a sensible size, and the display name is cleaned and given the extension of the real type.
Private serving: a file is reached only through a signed-in route that checks the centre, and an original is sent as a download with a fixed type and no caching. No storage address is ever sent to a browser.
A viewer with window and level, zoom, pan, frame scrolling, invert and keyboard control, in right-to-left and left-to-right pages. It states that it is not a diagnostic workstation.
Header identity kept apart: a mismatch with the chart shows staff a warning and never links anything, and images of an unconfirmed file cannot be released to the patient until a doctor confirms it.
Storage: the plan's storage allowance is reserved before a file is written. When it is full, or when private storage is not available, the upload is refused and nothing is stored.
Report integrity: one current version, signed versions that cannot change, a check that you sign what you were looking at, and an amendment that needs a reason.
Release: once per version. An amendment is not released automatically, so the patient stops seeing the replaced text at once. Files cannot be added or removed after a version is signed.
What the viewer opens and what it only stores
These are the cases the viewer is built and checked for. Everything else is kept, with the reason shown and the original available to download.
- Opened: DICOM with Implicit VR Little Endian, Explicit VR Little Endian or Explicit VR Big Endian (uncompressed).
- Opened: DICOM with JPEG Baseline (8-bit) compression, including multi-frame files with one fragment per frame.
- Opened: 8-bit and 16-bit greyscale, signed or unsigned, with the header's rescale and window applied, and 8-bit colour; PNG and JPEG pictures through the browser's own decoders.
- Stored, not shown: deflated, RLE, lossless, extended and progressive JPEG, JPEG-LS, JPEG 2000 and video compressions; palette colour, 1-bit and 32-bit data; files without pixel data or with truncated data; images over 16384 pixels on a side.
- Not accepted at all: SVG, HTML, PDF and any file that is not DICOM, PNG or JPEG by its content.
Setting up and moving over
What you need before it works
- A private file store connected to your deployment. Until then every upload answers with a clear error and nothing is stored.
- Switch the Imaging module on in Settings, organisation. It is on by default only for an organisation registered as an imaging centre.
- Give each person who registers studies and uploads the assistant role, and make sure a user with the doctor role exists: a solo practice's owner account cannot sign a report.
- Make sure private file storage is configured for your deployment; without it the app refuses uploads instead of storing files elsewhere.
- Decide whether doctors may release images to patients. The switch is on the Imaging page, off by default.
- Keep each file under about 4.5 MB because of the hosting platform's request limit, and use a Chromium-based browser on a desktop computer.
Limits worth knowing
- No PACS, DICOMweb (STOW, QIDO, WADO) or modality connection, and no modality worklist: files are exported from the equipment and uploaded by a person.
- Not a diagnostic workstation: no measurements, annotations, 3D, MPR or CBCT, and no comparison of studies.
- Nothing is read automatically. The viewer shows files as stored, and every word of a report is typed by a doctor.
- The application accepts files up to 25 MB, but the hosting platform refuses a single request above about 4.5 MB before the application sees it. Large studies cannot be uploaded today, and uploading straight to storage is not built.
- Only Chromium on a desktop computer has been tried, with synthetic files. Safari, Firefox, files from real CT, MRI, radiography and mammography equipment, very large multi-frame files, and the hosted private store with this module have not.
- No retention or deletion policy for studies (a file can be removed only before signing) and no button to withdraw a release; amend the report instead.
Questions
Which DICOM files will open in the viewer?
Uncompressed files (Implicit VR Little Endian, Explicit VR Little Endian, Explicit VR Big Endian) and JPEG Baseline 8-bit files, single or multi-frame, in grey or colour layouts the viewer lists. Other compressions such as JPEG 2000 or lossless JPEG are stored but not shown.
What happens to a file the viewer cannot open?
It is kept with the study and shows why it cannot be displayed, with a link to download the original and open it in another DICOM viewer. A deflated DICOM file is never expanded by the application, as a precaution, so its contents are not read at all.
Can patients or other staff see the name inside the DICOM file?
No patient can: the portal never reads those fields. Staff see a warning only when the header does not match the chart, and the header never decides which patient the study belongs to.
Who can sign and release a report?
Only users with the doctor role. An owner or assistant cannot, and reception cannot open imaging. A signed version never changes; a doctor amends it with a written reason.
What are the size limits?
A study can hold up to 200 files. Each file counts against the plan's storage allowance, and the upload is refused when the allowance is full. Because of the hosting platform, one upload request is limited to about 4.5 MB.
Can patients download the images?
Only when the owner has switched image release on, the doctor chose it for that release, the study has files, and any file with a mismatched header identity has been confirmed by a doctor. Switching the setting off removes the patients' access to images at once.
Does it read the images or flag anything for the doctor?
No. There is no automatic reading and no measurement, and nothing is suggested to the doctor.
